What is the cost?

$95 for Certification

$150 State fee (to be paid directly to ADHS) or $75 with EBT card.

You must have an active EBT card with your name printed on the front or raised like a credit/debit card. If that is not present on your card, or someone else on your account has their name on the card, we will need the most recent approval letter.

What form of payment do you take? We accept the following cards (Master, Visa, Credit and Debit)

Where are you located? We are an online based company. Everything is handled through Telehealth.

What type of medical records do I need? Any type of medical records from your doctor within the past 12 months, such as any chart notes or progress notes indicating that you have 1 of the 13 qualifying conditions in the state of Arizona. We can help you obtain your medical records with a printable medical release form that can be filled out and emailed to your primary doctor.

Does Insurance cover medical marijuana? Unfortunately, as of right now Insurance will not cover medical marijuana in any state due to the fact that marijuana is still illegal federally.

What type of physicians are allowed to approve patients for medical marijuana? Allopathic (MD), Osteopathic (DO), Homeopathic [MD(H) or DO(H)], and Naturopathic [NMD or ND]. Physicians who have a relationship with the patient may write certifications for medical marijuana. The physician must hold a valid Arizona license. 

Do I need a medical card to purchase CBD? No, you do not. CBD is non-psychoactive; therefore, a medical card is NOT required in order to purchase CBD. 

How long does it take the state to send my marijuana card? 1-5 business days.

If I am a convicted felon with a past drug possession charge, can I still receive a medical card? Yes, as long as you are approved by an Arizona licensed physician.

Can women who are pregnant use Cannabidiol (CBD) , Tetrahydrocannabinol (THC), and Marijuana? FDA strongly advises against the use of cannabidiol (CBD), tetrahydrocannabinol (THC), and marijuana in any form during pregnancy or while breastfeeding. Cannabis and Cannabis-derived products have become increasingly available in recent years, with new and different types of products appearing all the time.
